1. A request should pertain to specific documents and/or records that Sullivan County maintains. See below section, “Does Not Require A Right To Know Request” for records that do not require a Right To Know request under the Right To Know law.

2. A Right To Know request must include the above request form completed with the initial request. We cannot process or accept requests without this form present.

3. Completed request forms can be sent via mail or email to the address in the contact section below.

Does Not Require A Right To Know Request

The following documents do not require a Right To Know request to be obtained:

• Birth and/or Death Certificates

• Deeds

• Wills

• Marriage Licenses

• Environmental Records/Hazardous Materials/Permits for Codes and Zoning

• County Contracts

• Tax/Property Assessment Documents

• Bid Information

• Criminal Cases

• Civil Cases
