1. A request should pertain to specific documents and/or records that Sullivan County maintains. See below section, “This address is not to be used for the following” for records that do not require a Right To Know request under the Right To Know law.

2. A Right To Know request must include the above request form completed with the initial request. We cannot process or accept requests without this form present.

3. Completed request forms can be sent via mail or email to the address in the contact section below.

This address is not to be used for the following:

• Copies of Birth and/or Death Certificates

• Copies of Deeds

• Copies of Wills

• Marriage Licenses

• Environmental Records/Hazardous Materials/Permits for Codes and Zoning Information

• County Contracts

• Tax/Property Assessment Records

• Bid Information

• Criminal Cases

• Civil Cases
